Create a 1- to 2-page patient educational handout for the Leading Health Indicator you select from the following link https://www.healthypeople.gov/2020/Leading-Health-Indicators. Use the Community Guide website to find a population health program that focuses specifically on the LHI that you selected. This program should include information on the financial aspects of providing the program as well as participating in the program. This handout will be patient-focused and should be written at a sixth-grade level. The goal is to identify and manage or prevent chronic illness. You can use pictures, graphs, charts, etc. to make your handout eye catching and get the point across. Be sure your handout is easy to read and can be understood by the general population. If you use images, make sure these are not copyrighted and are culturally diverse and appropriate.

  • identify the specific population that you are addressing.
  • Describe the needs of the population.
  • Identify the LHI that this population health program addresses.
  • Describe at least two social determinants of health that impact the population.
  • Identify and briefly describe the population health program that you selected.
    • Explain whether or not the program gained success within the population.
    • Include an APA reference to the article.
  • Research the economic cost of the disease or condition that is being addressed and include that information in your handout.
  • Research the costs of implementing this program (if available), or programs in communities that are similar in size and demographics and include this information in your handout.
  • Describe the economic benefits of participating in the program.
  • Explain what measures are used to determine the success of the program. Include data that pertains to the measurement of success. If no data is included in the article that pertains to success measurement, indicate that on the handout.
  • Use CBA, CEA, ROI, or QALY to evaluate the success of the program. Each of these analysis tools is described in the resources. Explain why you chose the method that you used.
  • Based on your CBA, CEA, ROI, or QALY analysis, do you think that this program has the potential to impact the population positively from an economic perspective?
  • Be sure to cite and reference at least three supporting resources at the end of your document.
